Coming home from hospital can be a worrying time. We provide personal care and practical support at home in Sandwell, with care that can often start within a few days.
The sooner we know the planned discharge date and the support needed, the better we can plan. Tell the ward discharge team that you want care at home, then call us with the date and what help the person is likely to need.
Care after hospital discharge is often short term while someone recovers, or it can continue longer if needs have changed. We agree a plan with you and adjust it as the person regains strength.
After a hospital stay, including after a stroke, many people need help with washing and dressing, preparing meals and drinks, moving safely around the home, medication prompts, and company while they settle back in.

We support people returning home from hospitals including Sandwell General Hospital in West Bromwich and Russells Hall Hospital in Dudley. Ask the discharge team what short-term support the NHS or council can arrange, and contact us if you would like to arrange private care or continue after that support ends.
The first step is a free, no-obligation consultation. We visit you at home, listen to what matters to you, and build a care plan with you and your family. We then introduce one or two carers chosen for compatibility, not just availability. For most clients, care can begin within a few days of the consultation, and sooner in urgent situations.
Other ways we can help: Personal care at home · Dementia care · Care for older people · Respite care · Physical disability support.
Areas we cover: Tipton, Sandwell, West Bromwich, Wednesbury and Dudley.
